  • To receive accurate and easily understood information about your health plan, healthcare professional and healthcare facility.
  • To get a copy of the Notice of Patient Privacy Practices.
  • To file a complaint if you feel your rights are violated.

    • You can file a complaint with the U.S. Department of Health and Human visiting www.hhs.gov/ocr/privacy/hipaa/complaints/.
    • We will not retaliate against you for filing a complaint.

Patients' Responsibilities
