Though not all of us are sure what sports may look like for our students, we do know that physicals will still be required!
When students receive their sports physical at Xpress Wellness Urgent Care, $10 of their visit will be returned to their school’s athletic department.

**Students must return this form to their school by September 30, 2020. School representatives should mail all completed forms in one file to their nearest Xpress Wellness Urgent Care location or submit via e-mail to wecare@xpwell.com by October 16, 2020.**
