Facility Based Medicine
The national average readmission rate for skilled nursing residents returning to the emergency room is approximately 25%. Research indicates that up to 60% of long-term care nursing home residents are readmitted to the hospital at least once within a 12-month period. At Xpress Wellness, we are committed to implementing effective processes aimed at reducing these statistics.
